The Personal Assistance Services Program (PASP)
PASP provides routine non-medical assistance to adults with disabilities who are employed, involved in community volunteer work, or attending school. Personal assistants help with tasks such as light house keeping, bathing, dressing, preparing meals, shopping, driving, or using public transportation. This program differs from many other personal care programs in that people with disabilities direct their own services with an eye toward more productive and fuller participation in the community at large. Participants prepare a monthly budget to indicate how the cash value of their program services will be used and SDS, as the third party fiscal intermediary implements that budget exactly as written using the participants' program funds.

Accounts Payable Schedule 2023
If you have elected to have a small portion of your grant used for non-payroll expenses each month, you can use the non-payroll A/P check schedule to find out when those checks will be going out each month. It may be any Friday in the month, so you really have to check the schedule.

Cash Management Plan (CMP)
The Cash Management Plan CMP is your monthly budget. It determines exactly how the cash value of your services will be used. SDS makes no administrative or managerial changes to the CMP, we implement it exactly as written unless and until you write a new one. Each CMP must be submitted to the state for approval, after which the state will submit it to us with their approval for implementation. You may not overspend the budget at any time. There is no requirement for revisions of the CMP. You may go along with the same CMP for years as long as it still works for you. The only requirement is that it be balanced, not overspent and be approved at the state level. The CMP is an official document, developed by the state that defines the way in which you will use his funding. Without a CMP, no services can be provided.

This Form is to be filled out in its Entirety, Signed and Dated by Potential Employees. Once this application is complete it is your responsibility to Review it, Conduct Interviews, Check References, and make appropriate Employment Decisions. If you decide to Hire an Individual you must make sure all of the required paperwork as indicated on the Employment Cover Sheet Document (above) is complete before mailing to us for processing at the address on the form.

This form is to be filled out in its entirety by all SDS assistants. This questionnaire will be used to determine whether you are eligible for certain tax exemptions due to your relationship status with your participant. please fill in the date, your name, address, phone number, and date of birth in the appropriate spaces. Next, please fill in the name of the SDS participant for whom you will be working, if you know the pin you may fill it in but if you do not we will fill it in when we get the form back. Finally, please answer the question as to the participants relation to you, if any, you do this by checking the appropriate box. Sign, date and print your name on the spaces provided and have your participant (your sds employer) do the same. Remember, to be valid, this form must bear the original signature of you and of your SDS participant (employer).
USCIS I-9 Immigration and Naturalization Employment Eligibility Verification Form
Form I-9 is used for verifying the identity and employment authorization of individuals hired for employment in the United States. All U.S. employers must ensure proper completion of Form I-9 for each individual they hire for employment in the United States. This includes citizens and noncitizens. Both employees and employers (or authorized representatives of the employer) must complete the form.

This form is for your Employee to complete and return to you. It is for you to keep in case of an emergency in which you will need to notify the Employees Friend/Family. Please keep it in a safe and accessible place in your home. Please have your employee complete all information on the attached employee notification form.
Pre-Employment Background Check Permission Instructions
These instructions will help in completing accurately the Pre-Employment Background Check Form.
Pre-Employment Background Check Permission (This is Optional)
You may decide that you would like to have a background check completed on a potential employee. If you decide you would like to have a background check conducted on a prospective worker, please select the type of background check you would like conducted from those listed on the attached page. You must also have your potential employee complete and sign the consent form attached. Background checks are an optional service offered by SDS Fiscal and you will be billed for this service. If you have any questions about background checks or when they may useful please contact your consultant or the information and referral counselors.
Statewide Criminal Check -This background check will provide you with any information about criminal convictions in the State that you search. If a potential employee has just moved from another State, you would want to search that state instead of New Jersey. A Statewide Criminal Check will uncover criminal convictions for the entire state searched. It takes 2-3 Weeks.
Employee Time Sheet Instructions
Employee timesheets are utilized for a two-week period, Timesheets should be mailed to us immediately at the end of the two-week period. Please consult the pay schedule for the actual dates of pay periods. These instructions will help the completion and processing of your timesheets accurately.
Employee Time Sheet Form
Employee timesheets are utilized for a two-week period. It is recommended that the employee keeps the timesheet at your residence and fills it in at the end of their shift. This way you may both review the hours worked for that day, rather than waiting two weeks to review it.
Timesheets should be mailed to us immediately at the end of the two-week period. Please consult the pay schedule for the actual dates of pay periods.
Timesheets must be completed in their entirety. You must mail the original to us; copies and faxes will not be accepted. It is recommended that you retain a copy of timesheets submitted, for your files.
The employee must initial all changes made on the timesheet (absolutely NO White Out).

Note: You will be assigned a participant ID # by SDS Fiscal. If you have not received notification of this number by the time you are ready to submit your first timesheets, please contact us.

Authorization Agreement For Automatic Deposits Instructions
This document will help you complete the Direct Deposit Authorization Agreement Form, this form is for the employees only. We will not accept any other form except the one we have provided for you.

This form is for the employees only. We will not accept any other form except the one we have provided for you. If you choose to have direct deposit into your checking account, please attach a blank check from that account, make sure it has your pre-printed name on it, write void across it and mail to SDS Fiscal attached to your completed form. If your checks do not have your pre-printed name on it, you may attach a blank check with void written across it along with a copy of your bank statement for that account and mail with your completed form to us. If you choose to have direct deposit into your savings account, please bring the attached direct deposit form to your financial institution to have the bank teller fill it out and stamp it with their bank stamp for verification purposes.
Please note: The first pay date after we receive your Direct Deposit Form in our office will result in a pre-note. This means that we will process a deposit using a zero balance in order to ensure everything will go through correctly. The first pay period after receiving the form you will receive one more pay in the form of a check. This process will make sure that your pay is not delayed in case of a mistake with the bank information. This is a bank requirement and cannot be avoided.
Please be aware that your monies will be transferred into your designated account on the Monday following payroll. As Fiscal Agent for your employer, we do not accept faxes, copies of timesheets, unsigned timesheets or incorrect timesheets. We will return them for your correction/completion and there could be a delay in processing. If your timesheet arrives later that Monday of a payroll week due to a delay from the post office or any other reason, we will not be able to process it until the following payroll. Therefore, it is up to you to make certain that we receive your correct and complete timesheet during the designated dates we process payroll. There is a payroll schedule in your employment package but if you require an additional schedule or have any question regarding direct deposit, please contact the information and referral counselors at the phone number below.

Family Leave Notice
Under NJ Temporary Disability, eligible workers may apply for NJ Temporary Disability benefits to bond with a newborn or a newly adopted child, or to provide care to a seriously ill family member.
